A structured, six-stage investigation process, designed to be thorough, defensible, and focused on producing the learning that prevents recurrence.

Initial Response & Scene Preservation

Rapid deployment to secure the scene, preserve physical evidence, and initiate witness accounts before memories fade and conditions change. We advise on immediate notification obligations and regulatory reporting requirements.

Evidence Gathering

Systematic collection of physical evidence, documentation, photographs, CCTV, maintenance records, risk assessments, training records, and management system documents — building a comprehensive evidential foundation for analysis.

Witness Interviews

Structured, trauma-informed interviews with witnesses, supervisors, and managers — using validated cognitive interview techniques to elicit accurate accounts whilst maintaining the dignity and wellbeing of those involved.

Causal Analysis

Applying structured methodologies — including root cause analysis, bow-tie, ICAM, or human factors frameworks — to map the causal chain from immediate events back to the underlying organisational and management system failures.

Report & Recommendations

A professionally written investigation report with an executive summary, detailed timeline, evidence log, graded causal factors, and specific, prioritised recommendations — structured to be credible to regulators and actionable for operational teams.

Learning & Follow-Through

We present findings to leadership, support the development and tracking of action plans, and can provide follow-up verification that critical recommendations have been effectively implemented — closing the loop between investigation and improvement.
